Service Description
Each visit is tailored to help you meet your weight loss goals. Your current health history is factored into why weight loss has been difficult. Also (if necessary) a prescription for labs: CBC, CMP, THYROID PANEL, A1C, VIT. D, AND HORMONE PANEL (covered by insurance thru Quest). Our healthcare provider will review your results and can provide prescriptions for deficiencies in collaboration with your primary care provider. ************************Medication is at an additional cost****************** WITHOUT OUT MONTHLY SUBSCRIPTION: Injectable once a week weight loss GLP-1 medication starting at $180-$310. Injectable once a week stronger combo GLP-1/GIP starting at $190- $870 MONTHLY SUBSCRIPTION OF $100 INCLUDES: GLP-1: starting at $125-$225 GLP-1/GIP: starting at $120-$770 With subscription services you also get the following: *Your provider can complete a Prior Auth in an attempt to have your insurance cover the cost. If your insurance does not cover the name brand medications you still have access to our discounted drug costs. * Discount meals through our partnership with Trifecta
Cancellation Policy
To reschedule or cancel your appointment 24hrs before your appointment to avoid a $25 no show fee. If this fee is assessed it must be paid in full prior to your next or rescheduled appointment. Our provider has a right to deny an appointments and or services based on non-payment or no show fees and or copays that have not been paid. Also services may be declined or patient will be discharged from this practice if the patient in non-adherent to medical advice and or treatment plan causing exacerbations of their health and or causing negative outcomes to their wellness. If a patients behavior puts staff safety in jeopardy, the patient will be discharged immediately from this practice. We do not provide: -30 day supplies or controlled pain medications. Patients would need to follow up with pain management. -Prescriptions on Adderall or any other control for ADHD or any other diagnosis associated with medications in this class.
